full fact have raised exactly these concerns many time (but before their website was redesigned, so nothing there right now)

See table 1.

Even this confirms that in 10% of new claims decisions (ie including the more than 50% of DLA new claims that were not sucessful) the main (but not only) source of evidence used was the claim form.

So when the government said claim form they failed to let the public know the claim form can include other evidence ie it is not all just the answers the claimant writes down.
